It is very important for medical staff to be as aware as possible of the situations from which presenting medical problems are coming. Staff working in the prison environment should have some familiarity with the general literature on prison culture and should supplement this by involving themselves in the daily life of the establishment in which they are working.
This involves being part of the life of the establishment, being seen around all parts of the prison, chatting to people of all grades and status and being aware of their concerns. It is also appropriate for medical staff to play a role in working groups and management groups where there is a need for some medical input. But the doctor's contribution should not be limited to strictly medical matters. Obvious examples are health and safety, catering, general hygiene, drug and alcohol abuse programmes and mental health promotion.
All health promotion programmes provide an easy entree into stress reduction and suggestions for changes which might promote a safer environment.
